  • I have read, understood and accept the terms and conditions of the 13th Month Pay Support Programme ("the Programme") as specified in the application form and the FAQs, which can be found here.
  • The information I have given in this application is true and correct to the best of my knowledge and belief.
  • The employee(s) that are subject to this application is/are entitled to receive 13th month pay pursuant to section 46C of the Employment Act 1995.
  • Any payment that is made under the Programme will be passed on to eligible employees.
  • I will inform the Ministry of Finance, Economic Planning, Trade (“the Ministry”) if any information contained in this application changes.
  • I will furnish any additional information relating to this application if required or requested to do so by the Ministry.
  • I understand that the submission of this application does not amount to any commitment on the part of the Ministry to make payment under the Programme.
  • I understand that, if I give false, misleading or incorrect information, this application may be refused.
  • If it is discovered by the Ministry following any disbursement of payment under the Programme that false, misleading or incorrect information was provided, or that the moneys were not used to pay employees under the Programme, the applicant business will be required to repay any such moneys to the Ministry immediately.
